You must have a referral from one of the following sources:
- Your primary physician
- The primary treating physician (if Worker's Compensation)
- Your consulting physician
- Your attorney
- Your nurse case manager/ medical case manager
- Your employer (Worker's Compensation)
What insurance plans are accepted?
Many insurance plans are accepted. Please inquire about your plan when scheduling an appointment.
Does Dr. Davis take Worker's Compensation insurance?
Yes. In most cases, Worker's Compensation insurance is accepted. However, you must have an approved referral from your primary treating physician, medical case manager, attorney, or employer. Worker's Compensation patients are seen as consultations only. Your primary treating physician must continue your disability forms until the day of surgery, when Dr. Davis will take over as primary treating physician for ninety days.
Why do I need a referral from my primary physician, if my insurance lets me go to anyone without authorization?
In nearly all cases, you must have had an initial work-up by a physician, and the work-up must find an indication that surgical consultation might be necessary. Alternatively, you may have a known surgical condition and need a second opinion, which your primary physician can request.
What can I do to try to get an appointment with Dr. Davis sooner?
Some patients have dire conditions that need urgent surgery. Your primary physician can call Dr. Davis to request an urgent consultation either as an out-patient or as an in-patient in the hospital. The MRI scan and a description of symptoms is critical for this assessment. Every effort will be made to accommodate urgent requests.
What can I do to get the most from my appointment with Dr. Davis?
You can prepare for the visit by filling out all the required paperwork in advance, including the admission form and the medical questionnaire.
Bring your MRI, X-rays, CT scans and all other imaging studies. These often come from different providers. Do not assume they are all "in the packet." So, bring all reports for those studies, your doctor's notes on your condition, and any other important information (such as the operative note, if you have had prior surgery).
Wear clean, loose clothing that is easy to remove or adjust to facilitate the examination.
Do not smoke just prior to the visit.
Do not bring children unless they are the patients themselves.
If your English is not perfectly fluent, consider bringing a friend or relative who can act as interpreter to facilitate your understanding of the doctor's findings.
